Photo Release Forms - Commemorative Air Force

Those attendees who want their photo taken must fill out and return two forms:  A is for PERSONAL use, and B is for the Photographer.  The forms must be filled out and returned no later than January 3^rd.    Please use JCCA as the Organization Name on the form since that is the name of our Group.
